Relevant extract of the said e-mail dated 16.01.2025 reads as under:- “Dear Sir/Madam, References have been received with regard to the review/revision of Annexures ‘C’ & ‘D’ i.e., the format for seeking parental consent required to be submitted with the Passport Application of minors. 2. The format of Annexure ‘C’ and ‘D’ has been revised. A copy of the same is attached to this email. The revised format is being uploaded on the PSP/GPSP System and Passportinida portal, which is scheduled to be done by 20th January, 2025. 3. All PIAs and officials handling passport work may take note of the revised Annexure ‘C’ & ‘D’ for implementation once uploaded/from 20th January, 2025 onwards. However, for the initial 1-2 weeks certain relaxations may be given and existing as well as revised formats may be accepted, as brought by the applicant. This is to avoid inconvenience to such applicants who have submitted their online applications and have prepared documentation before 20th January, 2025. Concerned officers may take additional measures, as appropriate for smooth implementation of the revised Annexure ‘C’ and ‘D’ ad reconcile any grievance on this at their level suitably. 4. The related provisions/instructions have also been revised and sent to all RPOs/Missions/Posts and you may receive these within a week.
